- Information re when you are available and the type of locum you prefer (long/short term etc)
- Name and contact details
- Current CV - this should have no unexplained gaps in it, with start and end dates of all appointments.
It should include a full list of procedures you have and currently undertake and full details of your
qualifications.
- Names, full addresses with contact details (including ideally email for speed) of 3 professional referees.
- GMC Registration Number
- Enhanced CRB Disclosure - we will need to get you one in our name but if you have a current one

- We will need you to fill in the Occupational Health Questionnaire and return it to us with the necessary
reports
- If you work overseas we need a Police Clearance Certificate from your own country
